Though the database is designed such that articles may have multiple enclosures, picoFeed only exposes one---presumably the first found.

While this is in keeping with implicit RSS rules, Media RSS allows for multiple enclosures, as does Atom. Significantly, Tiny Tiny RSS also allows for the reporting of multiple enclosures.

Ideally Media RSS should be supported along with more mundane enclosures.
